Depressive and Anxious Tendencies in Pediatric Migraine Patients

Abstract
Purpose: The purpose of this study is to investigate depressive and anxious tendencies in pediatric migraine patients.

Methods: We retrospectively reviewed medical records of 196 patients who visited our hospital for headache and diagnosed with migraine according to the International Classification of Headache Disorders-III (ICHD-III) from March 2012 to December 2015. All patients completed the tests of the Children¡¯s Depression Inventory (CDI) and Revised Children¡¯s Manifest Anxiety Scale (RCMAS). We defined a CDI score of ¡Ã13 or ¡Ã15 and a RCMAS score of ¡Ã19 as clinically significant.

Results: The mean age of the patients was 11.9¡¾3.0 years. The ratio of male to female was 1:1.5, and 40.8% were male (n=80) and 59.2% were female (n=116). Degree of daily life impairments was significantly correlated with migraine frequency and intensity (P=0.041, 0.000, one-way analysis of variance (ANOVA)). Age was significantly correlated with CDI score (P=0.038, ANOVA), but it was not correlated with RCMAS score. Age was not significantly correlated with CDI or RCMAS scores in logistic regression analysis. The percentages of patients who had clinically significant CDI (¡Ã13 or ¡Ã15) or RCMAS (¡Ã19) scores were 26.0% (51 patients, Score of CDI¡Ã13) and 23.5% (46 patients, Score of CDI¡Ã15) for CDI and 31.1% (61 patients) for RCMAS.

Conclusion: The percentage of patients who had clinically significant CDI or RCMAS scores was higher than the range of prevalence rates yielded from studies in other nations that had also employed the CDI or RCMAS as an estimate of overall depression or anxiety levels. This result suggests that pediatric migraine patients might have more depressive and anxious tendencies than the general pediatric population.
